Return to view all job openingsKEY RESPONSIBILITIES/ESSENTIAL FUNCTIONS INCLUDE:
· Provide assessment and training activities for patients individually and/or in groups regarding daily living and communication skills based on a physician/optometrist’s plan of care. Record patient’s activities, monitor progress and modify as appropriate.
· Select activities that will help individuals learn work and life-management skills within limits of their mental and physical capabilities; help patients learn to perform various tasks such as using a computer, cooking, eating, dressing and exercising.
· Educate visually impaired or blind patients on lighting, contrast, eccentric viewing, magnification and optical devices.
· Train visually impaired or blind patients in the use of low vision devices, CCTV’s, and other adaptive technology/equipment as recommended by a physician/optometrist.
· Provide educational support to the families and caregivers of patients receiving vision rehabilitation services by communicating patient’s needs.
· Provide suggestions for assessment and instructional objectives to develop individual program plans for patients to the Medical Director and/or Director Rehabilitation Center.
· With visual impairments as outlined in their treatment plan of care.
· Responsible for timekeeping and attendance records for OTA.
· Complete all patient charting requirements according to the EMR software used.
· Apply instructional techniques to accommodate the unique sensory and physical abilities of patients with disabilities.
· Facilitate inter-agency cooperation on behalf of Envision and provide assistance and direction in determination of needed services. This includes monitoring local Emergency Preparedness programs and attending community wide exercises and facilitating tabletop exercises when needed.
· Perform research and collect data for physicians and/or clinicians as requested.
· Understand and practice compliance standards, including HIPAA confidentiality and security, and Medicare/Medicaid required documentation.
· Responsible for keeping up-to-date on current practices in the field through review of literature, attending conferences and seminars, continuing education requirements, AER certification, licensing and compliance needs.
· Perform monthly chart audits in compliance with Rehabilitation Agency policies and procedures
· Flexibility to perform other duties as assigned.
FOCUS Senior Outreach Program Responsibilities:
· Travel to assigned regions of the state of Kansas to provide occupational therapy assessment and training activities for patients.
· Initiate and maintain positive relationships with physicians/optometrists in assigned regions with the objective of 1) referrals back to this position, and 2) possible referrals back to the EDRC.
· Communicate role of Envision, and patient benefits under the Senior Outreach and Services program to physicians/optometrists and patients in assigned region.
· Increase patient services according to goals of the Senior Outreach and Services program.
· Maintain secure inventory of adaptive aides and optical devices while working at remote locations.
· Develop and facilitate a minimum of one monthly patient support group in collaboration with programming staff.
JOB REQUIREMENTS INCLUDE:
Education: Master’s degree in Occupational Therapy required.
Experience: 2 to 4 years previous experience as Occupational Therapist in the clinic or hospital setting required. Experience working with disabled populations preferred.
Knowledge/Skills:
· Ability to travel within State of Kansas to remote locations.
· Knowledge of and sensitivity toward the need for assistive technology and low vision aides for people who are blind of have low vision.
· Possess interpersonal skills necessary to provide close support and consultation to other professionals.
· Ability to demonstrate good patient relations in a positive, motivating manner.
· Must be sensitive to the patient’s issues and treat all patients with dignity, respect and kindness.
· Must possess the ability to communicate clearly both verbally and in writing.
Licenses/Certifications:
· Current Occupational Therapist (OT) license for state of Kansas required.
· Current National Board for Certification in Occupational Therapy (NBCOT) required.
· Current Certified Low Vision Therapist (CLVT) certified by the Academy for Certification of Vision Rehabilitation & Education Professionals (ACVREP) required, or be working towards this certification
· Current CPR/First Aid certification required.

___ Required to perform activities such as: operating machinery and/or power tools at or within arm’s reach; performing non-repetitive tasks such as carpentry work or repairing machinery.
