Practitioners serving individuals who are blind or visually impaired must ensure that they are equipped with the appropriate attitude, motivation, and skill set to provide effective training strategies that address the unique needs of individuals from culturally and linguistically diverse backgrounds. This knowledge based, two-hour virtual training is designed to help PACT members optimally provide care that is personalized, proactive, and patient-driven as part of an interprofessional healthcare team. There is a training gap focusing on the transformation from a disease-based model of care to a system that is focused on empowering and equipping Veterans to take charge of their health and well-being. Whole Health is personalized, proactive, patient-driven care that affirms the importance of the partnership between the clinical care team and the patient. The focus of the Whole Health clinical care visit has as its core 2 essential characteristics: (1) it emphasizes the patient s values which can be demonstrated by the question: What matters most to you? and (2) it empowers and equips the patient to take charge of his/her health and well-being. |
