 Description: Wondering how can APH Transition Hub support and increase the number of families that are directed to your summer residential program? Learn the latest innovations from APH CareerConnect. Join the National Transition Conversation as the APH ConnectCenter unveils the latest features of the Transition Hub project, and shares updates on CareerConnect. Just in time for summer programming across the United States, the National Transition Conversation will provide three mini forums spotlighting some of our partner transition programs and services. The second hour will provide a forum for practitioners, rehabilitation personnel, transition specialists and program administrators to share their own program successes and learn from other programs.
