Description of CE Program/Activity
 There are two videos included in this training module that discuss the importance of starting the process of transition to adult life as early as possible.Participants will:Differentiate the legal requirements related to transition planning.Distinguish the process of person-centered planning.Identify the benefits of person-centered planning.Recognize the importance of starting the transition process early in a students education.Examine the benefits of working as a team to facilitate the transition to adult life.
